First Visit at Sunshine Kids Clinic

To ensure we provide the best care during your child’s initial visit to Sunshine Kids Clinic, we kindly ask that you bring the following documents. 

Having these readily available will help us make the most of your appointment and gain a comprehensive understanding of your child’s health history.

Please bring the following:

• Referral letter from your GP or another doctor

• Medicare card

• Relevant reports (e.g. X-rays, MRIs, CT scans) and any other pertinent medical information

• Your child’s Blue Book

• Any previous health reports (e.g. blood tests, hearing assessments, speech pathology, physiotherapy, or occupational therapy reports)

• A copy of any MRI/CT scans on a disc or USB (if performed externally)
