First Visit

First visit at Sunshine Kids Clinic

To help us provide the best care during your child’s first visit to Sunshine Kids Clinic, we kindly ask you to bring the following documents. Having these on hand will ensure we make the most of your appointment and understand your child’s health history thoroughly.

Please bring

  • Referral letter from GP or other doctor

  • Medicare card

  • Reports, X-rays, MRI, CT scans etc. and any other relevant information

  • Your Child’s Blue Book

  • Any previous  health reports (Example :  Blood test, hearing ,  speech pathology, physiotherapy or occupational therapy, etc)

  • Copy of MRI/CT Scan on a disc/USB  if done externally.
